What this website collects
Only what you type into the contact form:
- Your name
- Your email address
- Your phone number, if you give one
- Your practice name, if you give one
- Your NPI, if you give one
- The message you write
Nothing else. This site sets no cookies, runs no analytics, embeds no advertising or social media trackers, and does not build a profile of you. There is no cookie banner because there is nothing to consent to.
Our hosting provider keeps standard server logs, including IP addresses, for security and diagnostics. We don't use those logs to identify individuals.
What we do with it
We use it to answer your enquiry, and to follow up about the services you asked about. That's all.
We do not sell, rent, or trade your information. We do not add you to a marketing list without asking. We do not share it with anyone outside the providers listed below.
Who processes it
Submitting the form passes your information through a small number of service providers acting on our behalf:
- Netlify hosts this website and runs the code that receives the form.
- Resend delivers the notification email to us.
- Airtable stores enquiries so we can track our response, where that service is in use.
These providers process the information to provide their service and for no other purpose. Each holds their own privacy terms.
How long we keep it
Enquiries that don't lead to a working relationship are deleted within 24 months. If we do work together, your records are governed by our client agreement rather than by this page.
You can ask us to delete your enquiry sooner at any time, and we will.
Your choices
Write to info@uhsmd.com and you can ask us to:
- tell you what we hold about you
- correct anything that's wrong
- delete it
- stop contacting you
We'll respond within 30 days. There is no charge.
Security
The site is served over HTTPS. Credentials for the services above are held server-side and are never sent to your browser. Access is limited to staff who need it.
No transmission over the internet is completely secure, which is precisely why we ask you not to send protected health information through this form.
HIPAA
When we provide billing services to a healthcare practice we act as a Business Associate under HIPAA, governed by a signed Business Associate Agreement with that practice. That agreement, not this page, covers how protected health information is handled.
This page covers only information submitted through this website, which should never include protected health information.
Children
This is a business-to-business website and is not directed at children. We don't knowingly collect information from anyone under 18.
Changes
If we change how we handle information, we'll update this page and the date at the top. Material changes will be noted here.
